    STATEMENT OF WORK (SOW) 1.0 Scope of Work: The objective of this project is to purchase and successfully install a new cone beam computed tomography (CBCT) imaging unit at Harry S Truman VA Medical Center. This includes ensuring the system is fully operational with the necessary software, training staff on its use, and providing ongoing technical support to maintain optimal functionality. 2.0 Background: The current Carestream 9000 imaging unit at Harry S Truman VA Medical Center has reached its End of Life and is beyond economic repair. In recent months, the unit has experienced frequent downtimes, resulting in numerous patient appointment cancellations and delays. This has significantly impacted patient care services and operational efficiency. The acquisition of a new brand name or equal Carestream 9600 unit is critical to restore reliable diagnostic imaging services. CONTRACTOR REQUIREMENTS 3.0 Technical Requirements/Tasks: The contractor shall: Deliver and install a new brand name or equal Carestream 9600 imaging unit at the specified location. Ensure system integration with existing IT infrastructure and ensure compatibility with current medical software. IE: VISTA imaging, CPRS and Future software Oracle. Provide comprehensive training for relevant medical and technical personnel on the operation of the new unit. Offer technical support and maintenance services post-installation to ensure ongoing optimal performance. For a Minimum of 5 Years. Base plus four. Maximum chin height of 67 IN and min height of 41 IN, chin rest must be able to move up and down between those heights Must weigh no more than 463 Ibs Must not take up anymore space than 59.06 (L) x 78.74 in (D) x 86.61 in (H) Generator tube voltage must be 60-90 kV Frequency must be at 140 kHz The focus for the image must be between 0.3 or 0.7 mm Image field must be 6.4 x 140mm Magnification must be able to do 1.28 Gray scale must be at least 14bits Radiological exams must have the following Full panoramic Segmented panoramic Bitewing Maxillary sinus Lateral tmj x2 Lateral TMJ x4 Sinus AP/PA/Lateral Orthogonal panoramic Exposure should be between 0.5 sec to 13 sec Tooth/teeth Full, upper or lower jaw TMJ Face ENT Upper cervical spine Input voltage for unit should be 50/60 Hz 100-240V Tasks: Delivery: Deliver unit to 800 Hospital Dr, Columbia, MO 65201. Ensure all components and software are included and in working order. Installation: Install unit as per the manufacturer s specifications. Perform all necessary calibrations and testing to ensure the unit is fully operational. Training: Conduct on-site training sessions for hospital staff on the use and maintenance of the new imaging unit. Provide training materials and user manuals. Support: Provide initial technical support post-installation to address any issues. Offer a maintenance plan for regular check-ups and emergency repairs (base + 4 option years) 4.0 Government Furnished: The Government will provide access to the required installation area within Harry S Truman VA Medical Center. Existing network infrastructure will be made available for integration with the new imaging unit. All reports and deliverables are subject to review and approval by the government. 5.0 Deliverables / Schedule: All written deliverables shall be phrased in layperson language. Delivery and Installation Plan:Â Within 10 calendar days from the contract award. Training Session Completion:Â Within 30 calendar days from the installation date. Maintenance and Support Plan:Â Detailed plan to be submitted within 15 calendar days post-installation. Deliverables should be submitted according to the specified formats and schedules. 6.0 Removal of Old Equipment: Contractor to remove and dispose of old equipment that is on-site (Carestream CS 9000). If trade-in credit available include discount information with quote. 7.0 Travel: Contractor personnel may be required to travel to the Harry S Truman VA Medical Center for installation, training, and support. The contractor is responsible for all travel expenses. 8.0 Contractor s Key Personnel: Key personnel essential for the successful performance of this contract include: Installation Specialist:Â Minimum 5 years of experience with installing medical imaging equipment. Training Specialist:Â Minimum 3 years of experience in training healthcare professionals on imaging systems. Technical Support Engineer:Â Minimum 5 years of experience in providing technical support for medical imaging systems. Resumes for key personnel should be submitted for approval. 9.0 Security Requirements: Personnel must follow VA security protocols and may need background checks and appropriate clearances to access the facilities and data. 10.0 Data Rights: Data produced, furnished, acquired, or used in meeting the contract requirements shall remain the property of the VA. The contractor must provide the VA with unrestricted rights to use and disclose any such data. 11.0 Section 508 Electronic and Information Technology Standards: All electronic and information technology must comply with Section 508 standards to ensure accessibility for people with disabilities.
