BPA FOR PHYSICAL THERAPY SUPPLIES / PRODUCTS FOR CHINLE PHYSICAL REHABILITATION DEPARTMENT

The Indian Health Service, Chinle Service Unit, Chinle Comprehensive Health Care Facility, located Chinle, Arizona 86503 has an upcoming requirement for direct patient care Physical Therapy (PT) supplies inclusive of PT miscellaneous supplies to be ordered on an ‘as needed’ basis as required by the Physical Rehabilitation Department, Chinle, Arizona.  Period of Performance is one year from the date of award. The North American Industry Classification System or NAICS code is 339113 Surgical Appliance and Supplies Manufacturing, with a small business size standard of 800 employees.  

The Request for Quotations No. IHS1507325 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 07/23/2025.  Evaluation factors may include technical capability to meet the government need, past performance, and price.  Quotes will be due on or by 07/23/2025 0800 a.m.  The acquisition is an Indian-Owned, Small Business Economic Enterprise (ISBEE) Set-Aside.

In accordance with 52.204-7(b)(1) prospective contractors must have – and maintain (in accordance with 52.204-13(c) – an active profile in the System for Award Management at beta.SAM.gov in order to submit a quote.
