DENTAL VACUUM PUMP SYSTEM REPLACEMENT AND INSTALLATION

The Indian Health Center, Inscription House Health Center located at Highway 98, Navajo Route 16 Shonto AZ, 86054
has a requirement for a dental vacuum pump and installation. The period of performance shall be one year from date of award. 

The Government will award the contract to the responsible offeror whose proposal represents the best value to the Government, considering evaluation factors such as technical capability, price, and product warranty, as detailed in the Section E of the solicitation document.

Quotation Submission Requirements

1. Price Quote (Required – Standalone Document)
The offeror shall provide a separate, standalone price quote document. The use of government forms (e.g., SF 1449) in lieu of a quote is not acceptable.
The price quote must clearly include, at a minimum:

• Item name/description
• Quantity
• Unit price
• Total amount

The price quote shall be presented in a clear, itemized format suitable for evaluation and incorporation into any resultant award.

2. Technical/Delivery Information (Section E Requirements)
The offeror shall provide a separate document, or include within the quote, a narrative addressing the requirements outlined in Section E, including but not limited to:
Technical Capability
Price
Product Warranty

3. Submission Format
All required documents shall be submitted as separate attachments or combined. The Government will evaluate quotations based on the information provided in these documents.

Failure to provide the required standalone price quote and Section E information may result in the quotation being considered incomplete for evaluation purposes.

The Government shall keep the solicitation posted and open for quotes until April 17, 2026 @ 12:00pm EDT.
