DLA TROOP SUPPORT HOSPITAL SUPPLY TAILORED LOGISTICS SUPPORT PROGRAM (TLSP)

Amendment 0005 was issued on April 27, 2026.

Solicitation number SPE2DH-25-R-0001 is hereby amended as follows:
The purpose of this amendment is to provide further clarifications and revise solicitation requirements. The following is a summary of the changes:

1. Attachment 5 - PEL Revisions (Amendment 0005)
* Section I: Clarifications – Provides clarifications based on vendor questions regarding TAA compliance, alternate item submissions, and details the specific items removed from the Price Evaluation List (PEL).
* Section II: Instructions for Price Submission –Contains updated pricing instructions and introduces the newly revised pricing spreadsheet: Attachment 1 - Price Evaluation Book (Amendment 0005).xlsx.

2. Proposal Submission Deadline (No Change): The proposal submission deadline is not extended by this amendment. The deadline remains May 5, 2026, at 3:00 P.M. EST, as previously established in Amendment 0004.

3. Revised Pricing Submission : The official pricing spreadsheet is hereby replaced with Attachment 1 - Price Evaluation Book (Amendment 0005).xlsx. The Price Evaluation List (PEL) is now reduced from 75 to 72 items, and the quantities for Line 1 have been corrected to reflect package counts. The following instructions are mandatory:
* Submission Tab: Offerors shall use the worksheet tab titled "PEL AMENDMENT 0005" for all price submissions.
* Reference Tabs: The original "Price Evaluation List (PEL)" and "PEL Amendment 0003" tabs are for reference only and shall not be used.
* Completeness Requirement: To be considered technically acceptable, proposals must include a price for all 72 items on the "PEL AMENDMENT 0005" tab, unless an acceptable alternative item is provided.

4. Attachments: This amendment includes the following attachments, which are hereby incorporated into the solicitation:
* Attachment 1 - Price Evaluation Book (Amendment 0005).xlsx * Attachment 5 - PEL Revisions (Amendment 0005).pdf

5. All other terms and conditions of the solicitation remain unchanged and in full force and effect.
