    The National Institutes of Health (NIH), Office of the Director, Office of Acquisitions, intends to negotiate and award a purchase order on a non-competitive basis to Dr. Abhay Moghekar, Johns Hopkins Hospital, 5200 Eastern Avenue #5100, MFL Center Tower, Baltimore, MD 21224, for neuropsychometric evaluations using the Clinical Dementia Rating (CDR) scale on participants in the Baltimore Longitudinal Study of Aging (BLSA), including scoring, rescoring, data entry and verification; preparation of summary information for joint NIA-Johns Hopkins consensus case conferences; and neuropsychological/neurological assessments on BLSA study participants between regular BLSA visits or not otherwise seen by the BLSA home visit program. Dr. Moghekar and his staff are uniquely qualified to perform this work based on their prior longitudinal experience with the BLSA cohort, their use of the standard LBN-BLSA neuropsychological battery, and Johns Hopkins University's role as holder of the Alzheimer's Disease Research Center (ADRC) grant in the Baltimore region. Transition to another contractor would result in at least a one-year delay in deliverables and redundant cost to the Government.
    (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2026-01, effective 03/13/2026.
    (iv) The associated NAICS code is 621111 ¿ Offices of Physicians (except Mental Health Specialists), and the small business size standard is $16.0 million. This requirement is not set aside for small business.
