PRESOLICITATION NOTICE (SYNOPSIS) The Indian Health Service, Gallup Indian Medical Center, located at 516 E Nizhoni Blvd Gallup NM 87301, has an upcoming requirement for Non-Personal Healthcare Services for Pediatric Physician Services – Outpatient, (2 positions). The projected period of performance will be as follows: Base period: 05/26/2025 – 11/25/2025 Option Period One: 11/26/2025 – 05/25/2026 Option Period Two: 05/26/2026 – 11/25/2026 Option Period Three: 11/26/2026 – 5/25/2027 Base period: 10/01/2025 – 03/31/2026 Option Period One: 04/01/2026 – 09/30/2026 Option Period Two: 10/01/2026 – 03/31/2027 Option Period Three: 04/01/2027 – 09/30/2027 Period of Performance dates may be subject to change. The North American Industry Classification System or NAICS code is 621111 with a small business size standard of $16.0 million. The Request for Quotations No. 75H71025Q00155 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13.5 Simplified Procedures for Certain Commercial Products and Commercial Services will be issued on or by May 2, 2025. Evaluation factors may include technical capability and approach in order to meet the government’s requirement as defined in Attachment A – Performance Work Statement, key personnel shall meet the requirements listed in section 10.0 of the Performance Work Statement, capability to meet the Period of Performance Schedule, past performance on three (3) projects providing non-personal service contract for Pediatric Physician Services-Outpatient within the past five (5) years, and price.
