This is a combined synopsis/solicitation for commercial products and commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a Request for Quote (RFQ) is being requested. Solicitation Number: 36C26324Q0581. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2024-02 dated 01/22/2024. This is an unrestricted solicitation meaning all business size may respond. At the time of submission of offer and prior to award, the offeror shall be active in the System for Award Management (SAM) reflecting 621111 Offices of Physicians (except Mental Health Specialists). The PSC Code is Q505 Medical Gastroenterology. The VA anticipates the award of one (1) firm fixed priced contract for On-Site Gastroenterology Physician Service to support the Des Moines IA VA Health Care System located at 3600 30th Street, Des Moines IA 50310. See Performance Work Statement and Price/Cost Schedule for specific requirements and qualifications. The period of performance is from June 3, 2024, to June 2, 2025. The contract is required to provide the following services: Board Certified Gastroenterology Physician Service, two (2) days per week, eight (8) hours per day, 104 days per year. This is a daily all-inclusive cost for the following services: (1) Inpatient/Phone Consult/Questions, (2) Unlimited Gastroenterology Procedures for the following: (a) GI EGD procedures (CPT: 43191-43278), (b) GI Colonoscopy procedures (CPT: 44388-45393), and (c) GI Sigmoidoscopy procedures (CPT: 45330- 45350). Contract physicians shall be capable of providing all required services. Solicitation Questions. Submission of Offer: Refer to FAR Provision 52.212-1 Addendum and 52.212-2 within the attached solicitation document for instructions on submitting an offer. Evaluation Factors. FAR 13 Comparative Evaluation. Offerors shall submit required documentation as outlined in the solicitation. Offer Due Date: Quotes are due to the Contracting Office by April 12, 2024, at 11:00 AM Central Time. Quotes shall be emailed to john.becker3@va.gov prior to the solicitation closing date and time. It is the responsibility of the offeror to ensure their quote is received in full by the Contracting Officer prior to the solicitation closing date and time.
