A current Drug Enforcement Administration (DEA) license, controlled substance license, and general license from the respective State Board of Pharmacy. All licenses must be in good standing. 6. Documentation showing pharmacists are in good standing with the Iowa State Board of Pharmacy. 7. Discount guarantees for brand name and generic drugs covering all ordering periods for this potential contract. Failure to provide this documentation with your proposal will result in the proposal being removed from competition.
