Fermi Forward Discovery Group (FFDG) anticipates procurement of the following for Fermi National Accelerator Laboratory (Fermilab). Please provide your firm fixed price for the following: Subcontractor to provide a firm fixed price on new safety footwear services for SAFETY SHOE FOOTWEAR (men and women) for a Base Year and 1 Option Year (Year 2): The vendor will provide ASTM F2413 compliant safety toe footwear to Fermilab employees at the Batavia, IL laboratory location as described below and listed in Schedules C: Safety Shoe Price Schedule. Medically prescribed corrections and occupationally required options for ASTM F2413 compliant footwear are listed in Schedule D: Occupational/Medical Footwear Price Schedule and must be available as an option when authorized. Footwear will be dispensed on the Fermilab site by the vendor. Dispensing will be conducted on every other Tuesdays (from 8:00 a.m. to 4:00 p.m.). The vendor will provide an adequate number of personnel to provide continuous service coverage for the entire eight-hour period. The dispensing facility will be a “shoe mobile” provided by the vendor. Reference the attached statement of work for complete details and pricing schedules (C&D).
