SHIPROCK SERVICE UNIT: PIV ACCESS CONTROL SYSTEM

The Indian Health Service, Shiprock Service Unit, located in Shiprock, New Mexico, 87420 has an upcoming requirement for purchase/installation of a new Personal Identification Verification (PIV) Card Access Control System to the Northern Navajo Medical Center in Shiprock, NM; Four Corners Regional Health Center in Teec Nos Pos, AZ; and Dzilth-Na-O-Dith-Hle Health Center in Bloomfield, NM 87413.

The period of performance will be as follows:

Base Year: 07/01/2025 – 06/30/2026

1st Option Year: 07/01/2026 – 06/30/2027

2nd Option Year: 07/01/2027 – 06/30/2028

3rd Option Year: 07/01/2028 – 06/30/2029

4th Option Year: 07/01/2029 – 06/30/2030

The North American Industry Classification System or NAICS code is 561621 with a small business size standard of $25.0 M Dollars. 

The Request for Quotations No. 75H71025Q00211 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13.5 Simplified Acquisition Procedures will be issued on or by 06/04/2025.  Evaluation factors may include:

1. Technical capability and Delivery/Schedule/Installation that meets the government requirements as described in Attachment A – Statement of Work or Specifications, particularly those minimum requirements identified in Section 3 Scope & Section 5 New Equipment Requirements & Section 7 Work Requirements.

3. Past Performance.  The Government reserves the right to retrieve and review past performance information from the Contractor Performance Assessment Reporting System (CPARS) and other federal sources and systems.   

Quotes will be due on or by 06/12/2025.  The acquisition is Competitive 8(a) set-aside.
