SOURCES SOUGHT-PEST CONTROL SERVICES FOR ZUNI COMPREHENSIVE COMMUNITY HEALTH CENTER (ZCCHC)

The Albuquerque Area Office (AAO) Indian Health Service (IHS) is issuing a Request for Information (RFI) notice for informational and planning purposes only. This notice shall not be construed as a solicitation, obligation, or commitment by the Government. This notice is intended strictly for market research to determine if requirements can be met through a 100% Set-Aside for Small Business concerns.

Purpose: The purpose of this notice is to identify qualified sources and their size classifications/socioeconomic status. This is a Request for Information (RFI) only. It is not a solicitation for proposals, proposal abstracts, or quotations.

Requirements:

• NAICS Code: 561710
• NAICS Description: Exterminating and Pest Control Services
• The requirement includes routine inspection, treatment and prevention of insects and rodents for hospital facilities and government housing quarters, as well as on-call and emergency response services.
• Location: Zuni Comprehensive Community Health Center in Zuni, New Mexico
  • (Similar NAICS code will be considered. Interested vendors proposing an alternate NAICS code shall submit a detailed explanation demonstrating how the proposed NAICS code aligns with and more accurately represents the principal purpose of this requirement)

Instructions for Respondents-Interested parties are invited to submit responses that include the following information:

   • Confirmation of registration and certification in the System for Award Management (SAM) at https://sam.gov.
2. Company Information
   • Company name, address, and point of contact
   • Unique Entity ID (UEI) number
   • Size of business under the identified NAICS code
3. Socioeconomic Status
   • Identify whether your business qualifies as:
     • Indian Small Business Economic Enterprise (ISBEE)
     • Service-Disabled Veteran-Owned Small Business (SDVOSB)
     • Woman-Owned Small Business (WOSB)
     • Other Small Business categories
4. Capability Statement
   • Provide a capability statement, product brochure, or relevant performance information demonstrating your ability to provide the required services.
5. ISBEE Representation Form
   • Complete and submit the Indian Small Business Economic Enterprise (ISBEE) representation form.

• Responses to this notice will be used by the Government to inform acquisition strategy.
• Interested parties must respond to any solicitation announcement separately from this RFI notice.
