    Description

    This is a combined synopsis/solicitation for items prepared in accordance with FAR Part 13. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.

    The solicitation is issued as Request for Quotes (RFQ) 246-25-Q-0078

    This procurement is 100% Indian owned set-aside under NAICS code 237110 with a size standard in millions of dollars $45.0.

    The Contractor shall furnish and install a water main extension at Greyhorse, Oklahoma as shown on the drawing and the statement of work.

    The period of performance for this requirement is 90 calendar days from Notice to Proceed.

    The official site visit is July 17, 2025 at 10:30 AM.  We will meet at the South end of Little Star Drive at the intersection of Little Star Drive and CR5451.  There is a map in attachments. Questions are due no later than July 23, 2025 5:00 p.m. CST. The closing date for receipt of quotes is August 12, 2025 COB CST. The offer shall be received by email to jeffrey.earnest@ihs.gov or in a sealed envelope, addressed to this office, showing the solicitation number, your name and address, no later than 5:00 p.m. CST, on August 12, 2025.

    THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO PROVIDE A PROPOSAL CONTAINING:

    A) Firm, Fixed price quote on the SF18 form and a completed attached bid sheet.

    B) Technically specifications of the statement of work, acceptable materials list to be used.

    C) (2) Relevant past performance of this type of work, shall provide organization’s name, point of contact, summary of project and award amount.

    D)Submit Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form (attached).

    E) Bid bond form (SF24) attached is required at $35,000.  If over $35,000 and selected as the acceptable vendor proposal, performance (SF25) and payment (SF25A) bonds shall apply.

    Award amount is estimated to between $25,000 and $100,000.

    Offers shall be evaluated on an acceptable/non-acceptable basis. Unacceptability of "A" and "B" (stated above) shall render the entire quote unacceptable.

    Evaluation shall proceed as follows:

    1. Quotes shall be ranked by price, from low to high
    2. Quotes shall then be evaluated for technical acceptability beginning with the lowest priced quote
    3. This process shall proceed until a technical acceptable quote is identified

    THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO MEET THE REQUIREMENTS IN THE STATEMENT OF WORK

    CONTRACT AWARD SHALL BE MADE TO THE RESPONSIBLE, RESPONSIVE OFFEROR OFFERING THE LOWEST REASONABLE PRICE AND MEETING ALL TECHNICAL SPECIFICATIONS.

    The technical evaluation shall be a determination based on information furnished by the vendor in the submitted proposals. The Government is not responsible for locating or securing any information which is not identified in the offer. The Government reserves the right to make an award without discussions.
