    Submit via email one copy of the Technical Proposal and Pricing Proposal as separate files. Cover page (Attachment 2) shall be submitted as page one of both the Technical Proposal and Pricing Proposal. Submission shall be made with the subject line “Response to RFP #26-11” to smchd.contracts@maryland.gov. Appended to your technical proposal shall be the proposers current license through the Maryland Office of Health Care Quality (OHCQ) under COMAR 10.07.14 to operate in St. Mary’s County, Calvert County and/or Charles County, disaster and relocation plan and outlined plan for all admissions. The St. Mary’s County Health Department is issuing this RFP 26-11 to realtors or housing specialists to provide educational support for community members of varied financial backgrounds on topics relevant to obtaining and maintaining stable housing. Housing instability, homelessness, and other housing-related challenges may contribute to poor health outcomes, including chronic health conditions and mental illness. Community members experiencing housing insecurity and homelessness may benefit from education and resources to support access to safe, sustainable, and affordable housing.
