The City University of New York (“CUNY” or the “University”) is seeking an actuarial firm to provide annual actuarial services on the retiree health care benefits for the University in accordance with Government Accounting Standards Board, Statement 75 (“GASB 75”). To be considered for Contract award, the Proposer and any Subcontractor(s) must meet the Minimum Mandatory Qualifications and certify such by signing Form 1 (see specific certification in Part III, #1, Form 1). Inability or unwillingness to meet the qualifications set forth below will result in the rejection of a Proposer’s proposal as non-responsive. Interested respondents are encouraged to review in detail the attached solicitation document and if the minimum qualifications are met to submit a proposal in accordance with the submission requirements as detailed within. Business enterprises awarded an identical or substantially similar procurement contract within the past five years: The Procurement Lobbying Act (PLA) Applies to this Solicitation. Compliance Procurement Lobbying Act Required Forms: Vendor shall complete, sign and submit the following forms if they are selected: 1. “Offerer’s Affirmation of Understanding of and Agreement pursuant to State Finance Law §139-j(3) and §139-j(6)(b)” 2. “Offerer’s Disclosure of Prior Non-Responsibility Determinations and Certification of Compliance with State Finance Law §139-j and§139-k”.
