The purpose of this Request for Bid (RFB) is to solicit competitive bids from qualified suppliers to provide and install network cabling for the schools listed hereafter for Canyons School District (CSD). It is anticipated that this RFB may result in a contract awarded to a qualified supplier for commodities and as defined further in the solicitation. This RFB is designed to provide interested suppliers with sufficient basic information to submit bids meeting minimum requirements, but is not intended to limit a bid's content or exclude any relevant or essential data. All bid preparation costs and any associated activities related to this RFB are the sole responsibility of the Supplier. When using Utah State Contract pricing, please reference contract number. All solicitation responses must be submitted electronically through Euna Procurement, powered by Ion Wave. All procurement-related inquiries relative to this bid must be submitted through the questions tab in Euna Procurement, powered by Ion Wave and will be answered through Euna Procurement, powered by Ion Wave up to the Q & A deadline. Proposals will be opened electronically within five (5) business days of the closing of the solicitation. MANDATORY ELIGIBILITY REQUIREMENTS In order to be eligible to respond to this RFB, an interested supplier must meet all of the following minimum qualifications: A. Licensed The interested supplier must be properly licensed to conduct business in the State of Utah. Please upload a copy of your license with your proposal. B. Non-Discrimination The proposer must comply at all times during the bidding and contracting period with all applicable Federal, State, County, and City anti-discrimination laws, ordinances, rules, and regulations. Any violations of this provision shall be considered a violation of a material provision of the audit contract and shall be grounds for cancelation, termination, or suspension. C. FCC Registered Service Provider The service provider must be an FCC registered service provider and be in good standing with USAC. D. E-rate Requirements The service provider must provide evidence that they have a Service Provider Identification Number (SPIN). The service provider must provide evidence that they have filed a current Service Provider Annual Certification (SPAC) (Form 473) and must maintain all Certifications. The bidding Supplier must be properly licensed to do business in the State of Utah. E. Termination Rights The contract shall provide that the Canyons School District has the right to cancel without cause at any time by written notice within thirty (30) days of its intent to terminate the contract. The Board of Education reserves the right to reject any or all proposals and to waive any informality or technicality in any proposal in the best interest of the District. Canyons School District may terminate the agreement in whole or in part for: · Insolvency · Material or persistent breach of the agreement. · Failure to meet the benchmarking/performance requirements. · Change of direct or indirect ownership or control of the supplier. · Breach of confidentiality, including data protection or security requirements. · Lack of E-Rate funding. The agreement should allow Canyons School District to terminate the agreement in whole or in part without cause or suitable notice. F. Contract Period This agreement shall commence no sooner than April 1, 2026 and after award of RFP and continue for one (1) year. E-rate E-rate eligible solutions are preferred but not required. However, vendors must include their USAC issued Service Provider ID Number (SPIN) as well as evidence that they have filed a current Service Provider Annual Certification (SPAC), form 473, in their proposal. Vendors must also include, to the extent possible, an assessment of E-rate eligibility for all products/services offered. Different but equal solutions will be considered. New solutions will need to maintain the current level of functionality, the determination of which remains solely with Canyons School District. If working within this mixed environment is not possible, eligible costs and ineligible costs will need to be included to replicate current functionality. References The proposal shall include a minimum of three (3) and a maximum of five (5) references over the past four (4) years. If possible, these should be from similar institutions, preferably from school districts. References should include information about the contract (specific products in use, date of contract execution, "go-live" date and specific services provided), as well as contact information from the client's project manager or other senior staff members familiar with the project. The District reserves the right to evaluate past performance obtained from other sources other than those furnished by the proposer. The proposer will be scored on E-Rate experience and eligibility. All bid responses will need to provide references. Emphasis should be given to providing references with similar composition. Size and scope, enterprise management, business purpose (education), and geographic proximity are examples of references that will be of particular interest. The supplier will list previous E-Rate experience, provide evidence that they have a Service Providers Identification Number (SPIN) and provide evidence that they have filed a current Service Provider Annual Certification (SPAC)(Form 473). NOTE: 1) It will be your responsibility to properly seal any firewall holes that you use. 2) If a scissor lift is required, you are responsible to provide. Also, responsible to cover floor(s) to protect. 3) It is our intent to award this RFB and contract(s) shortly after total purchase has been approved by Canyons School Board at the March 24, 2026 Board Meeting. Once done, a meeting will be schedule to discuss schedule and priorities. 4) The contract start date will be listed to start after April 1, 2026 5) Use the attached location maps, images and Cable Bid Doc for referencing the details of drops and patch panel locations. A1: Belden CAT6 plenum cable is acceptable and, as long as the Belden jacks used do not require a proprietary patch panel and will fit a standard HD blank patch panel, they are acceptable for this project.
