Alameda County Health (herein referred to as AC Health or the ‘County’) seeks to award contracts to qualified Bidders across Alameda County whose proposals conform to the Request for Proposal (RFP) and meet the County’s requirements. AC Health, Housing and Homeless Services (H&H) expects to award a minimum of six (6) contracts for an initial one (1) year contract term with an option to renew for an additional five (5) years. The actual funding award amount will be determined by the development of the service delivery models and financing plans proposed by the bidder and will be negotiated and finalized by H&H, in conjunction with the selected bidders during the contract negotiation period. Actual award amounts will be dependent upon the availability of funding and may therefore differ than those originally proposed in bidders’ response. Renewal of contracts with the selected Bidders is contingent upon meeting performance measures and contract deliverables, as set forth by the County and subject to periodic review and is dependent upon the availability of funding.
