    The City of Milpitas ("City") is seeking proposals from qualified vendors ("Proposer") in response to this Request for Proposals ("RFP") for services to perform backflow prevention assembly (BPA) testing and condition assessment by either a Cross-Connection Control Specialist or a BPA Tester accredited by an organization recognized by the State of California to perform testing and condition assessments on up to 171 BPA located on City-owned property at various locations.

      Has the firm satisfactorily described its ability to achieve budget, task and timeline delivery goals for projects of a similar complexity and nature as described in the Scope of Work? What is the firm’s management philosophy in accepting responsibility for project tasks or in sharing such responsibilities with City staff?

    • References (5 pts)

      Are the firm’s references from past clients and associates overall favorable? What is the Proposer’s record of deliverables submitted on time and within budget? Does the firm communicate with, collaborate with, and take direction well from City staff?

    • Personnel Assigned (20 pts)

      What are the qualifications of principals and project team members and what would be the primary responsibilities of each? What experience do these members bring with them in performing similar services or in managing subconsultants in such services? Do assigned personnel have the requisite education, experience, and professional qualifications?

    • Proposed Compensation (10 pts)

      Fee proposals shall include everything necessary for the completion of all Services or otherwise fulfillment of the Agreement including but not limited to furnishing all labor, materials, equipment, tools, facilities and all management; overhead expenses and profit required to complete the Services in accordance with the Scope of Work. Fee proposals must contain the information to accurately and completely capture all Proposer's costs.

      Time is of the essence for each and every provision of this Agreement/Contract.       The cover letter shall address the following items and be formatted as detailed below covering each item A through F. Failure to cover each item may be grounds for disqualification.
       
      1. The cover letter shall identify the legal form of the firm, and if a corporation, shall identify in which state the firm was incorporated.
      2. The cover letter shall be signed by a principal of the firm or other person authorized to act on behalf of the firm.
      3. The proposal shall identify the location of the firm’s home office, and whether or not the firm has management staff in the San Jose/Oakland/San Francisco area.
      4. The number of employees.
      5. Name, address, telephone number, and email address for Proposer’s point of contact for a contract resulting from this RFP.
      6. Length of time Proposer has been providing services described in this RFP. Please provide a brief description.
    • Company Profile (required)

      The Proposer shall provide a general description of the firm, including a brief history and its experience in providing similar services as those requested in this RFP. The City will only consider submittals from Proposers whose proposed team can demonstrate the following background and capabilities:

      • A long term, well established entity in the State of California;
      • Contractually commit that the team defined in the proposal will perform the Services under the Agreement should it be chosen.
    • Execution Plan (required)

      The plan represents the Proposer’s offer of Services to the City. The proposed execution plan must include a detailed work plan describing how the Proposer will meet the project objective in the most cost-effective and timely manner. This section is to provide a detailed explanation of the Proposer’s approach in performing the Services described in the RFP – SEE SCOPE OF WORK.

    • Personnel Assigned (required)

      Shall include all of the following:

      1. A project manager authorized to act on behalf of the firm must be designated and shall be the principal contact for the City.
      2. Identify any individuals expected to have backup responsibilities.
      3. Provide a brief resume/background of the specific individuals assigned to this project; resumes should not exceed one paragraph per person.
      4. The proposal shall state that no changes in key personnel are to be made without written consent of the City.
      Failure to comply with each item A through D may be grounds for disqualification.

      If the City elects to interview prospective firms, such persons shall be in attendance and materially contribute to the discussion.

    • Technical Proposal (required)

      Attach your Technical Proposal here.  Your Technical Proposal shall include, but not be limited to, the following:

      1. Confirmation of your ability to meet all required elements noted in the Scope of Work and any relevant details.
      2. Explanations of your ability to meet any desired elements noted in the Scope of Work.
      3. A list and samples of your standard, out of the box reports.
      4. Your recommended end-user and administrator training, including hours, curriculum, and maximum users per session.
      5. Your standard technical support hours, availability, methods, and SLAs.
      6. Your standard maintenance windows, guaranteed uptime, and service credits for failure to meet your guaranteed uptime.

      You may also include any other relevant information regarding your software Solution and/or Services you believe is relevant for the City to take into consideration in evaluating your Proposal.  Note: Do not include cost in this response.  All cost information should be provided in your Proposed Compensation response.

    • Proposed Compensation (required)

      This section shall consist of a detailed outline of Proposer’s proposed compensation for the products and services outlined in the Scope of Work.

      All proposals shall include the compensation proposed, both in writing and in figures, shall give all other information requested herein, and shall be signed by the Proposer’s authorized representative. Fee proposals shall include everything necessary for the completion of the work or fulfillment of the Agreement, including, but not limited to furnishing all labor, materials, equipment, tools, facilities, goods, and management; overhead expenses and profit required to complete the work in accordance with the Contract Documents, except as may be provided otherwise in the Contract Documents. Fee proposals must contain the information as outlined in the RFP.

      Note:  For any applicable ongoing services, any requests for annual increases will be capped at CPI or 3%, whichever is less.
