    Description

    The Hawaii County Office of Aging (HCOA) is requesting information from qualified applicants to provide coordination of the Better Choices Better Health (BCBH) Ke Ola Pono- Chronic Disease Self-Management Program (CDSMP) throughout the County of Hawai'i. Contracts are planned to begin on or after October 1, 2026. The purpose of this Request for Information (RFI) is to assist HCOA in obtaining community input for the development of Request for Proposals (RFP) for Coordinator of the BCBH Ke Ola Pono- Chronic Disease Self-Management Program. HCOA is specifically interested in feedback and comments regarding the items listed below. We welcome additional comments and suggestions. 1. Provide information on techniques or strategies to increase recruitment and retention of participants and lay leaders for the BCBH program. 2. Identify community partners, agencies, or stakeholders who could assist in promoting the BCBH program. 3. Describe experience needed to successfully monitor program fidelity, oversee program administration, measure quality assurance, and identify and resolve problems within program or service delivery. 4. Provide suggestions for other services and programs for older individuals or family caregivers that may be provided in the future. Written comments, suggestions, and questions are due by 4:00 PM August 3, 2026.
