SMALL PROJECT FUND REQUEST FOR QUOTES Project Name: Pizza Village Upgrade Initiative Project Address: 71 Lake Shore Drive, East, Dunkirk, New York 14048 Project No. SPF06 · Overhang Roof Replacements. Replacement of the existing overhang roofs includes commercial-grade roofing materials designed for durability and weather resistance. Work includes prep tarp and protect shrubs; inspect all sheathing and re-nail loose areas as needed. Sheathing rot replacement as needed using high-quality CDX 4 plywood. Remove the existing layers of flat roof, detach and reset the metal, and install the flat roof system. Install GAF Liberty Base Sheet and Cap Sheet; clean and remove all debris at project completion. GAF Manufacture warranty and two (2) years of workmanship in the repaired area. Compliance with all local building codes and regulations, including fire safety standards. Final inspection to certify the roof's structural sound and adherence to safety requirements. Asbestos-containing materials distributed as part of the project must be handled and disposed of in accordance with NYS Department of Labor requirements at 12 NYCRR Part 56 and local regulations. · New HVAC System Installation. Removal of the existing A/C system, old unit heater, and ducts. Set the air handler and ducted heat in place. Reconnect supply and return air ducts, gas, electric, flue vent, and drain. Install the condensing unit outdoors and connect it to the indoor unit using an insulated copper line set. Install a new disconnect and connect the electricity to the new unit. Replace (1) 8” supply and redirect from floor to ceiling. Cover the opening from the old A/C unit with plywood. Start-up and test operation of the new system. 1-year parts and labor, 5-year compressor warranty on Trane equipment. Compliance with all relevant codes, including environmental and energy performance codes. All work must be performed by licensed professionals in accordance with local building codes and regulations. All vendors must provide references and proof of proper insurance, including general liability coverage of at least $1 million and workers' compensation coverage. The City of Dunkirk, the State of New York, and the Housing Trust Fund Corporation must be listed as additional insured. Within 60 days of selection, the property/business owner will enter into a work contract with the lowest responsible vendor. If the property/business owner chooses a quote other than the lowest, reimbursement will be based on the amount of the lowest bid/proposal. Minority and Women-Owned Business Enterprises (MWBEs) and Service-Disabled Veteran-Owned Businesses (SDVOBs) are encouraged to submit quotes. An environmental compliance review conducted by LaBella Associates has determined that the project does not pose any health or safety concerns. Additionally, the review confirms that the project does not involve correcting any code violations and aligns with established local program design guidelines.
