      The Qualification Statement must be written in sufficient detail to permit the City to conduct a meaningful evaluation of the project team’s capacity to perform the services required herein, including the following:

      1. Specify the individual that will serve as Proposer’s lead representative who shall be responsible for all the work to be performed by the Provider under this Contract and shall serve as the point of contact.

      It has given the City written notice of all errors or discrepancies it has discovered in the Contract and the resolution thereof by the City is acceptable to Bidder/Proposer.

The price(s) quoted in the attached Bid/Proposal are fair and proper and are not tainted by any collusion, conspiracy, connivance, or unlawful agreement on the part of the Contractor or any other of its agents, representatives, owners, employees or parties in interest, including this Affiant.

      Bidder, in accordance with Florida statute 287.087 hereby certifies that the Bidder does all of the following:

      1. Publishes a statement notifying employees that the unlawful manufacture, distribution, dispensing, possession, or use of a controlled substance is prohibited in the workplace and specifying the actions that will be taken against employees for violations of such prohibition.
      2. Informs Employees about the dangers of drug abuse in the workplace, the business’ policy of maintaining drug-free workplace, any available drug counseling, rehabilitation, and employee assistance programs, and the penalties that may be imposed upon employees for drug abuse violations.

      5. Imposes a sanction on, or require the satisfactory participation in, a drug abuse assistance or rehabilitation program if such is available in the employee’s community, by any employee who is so convicted.
      6. Makes a good faith effort to continue to maintain a drug-free workplace through implementation of this section.
