    Description

    Washington County Public Health (WCPH) is seeking proposals from eligible organizations to expand access to mobile healthcare services and social supports in rural Washington County communities. This project supports implementation of the Washington County Community Health Improvement Plan (CHIP).


    Funding for this opportunity is provided through the Oregon Health Authority (OHA) Rural Health Transformation Program (RHTP). OHA receives this funding from a federal initiative administered by the Centers for Medicare & Medicaid Services (CMS). Organizations funded through this Request for Proposal (RFP) will work in partnership with WCPH to implement approved activities that support the goals of this project. This funding opportunity supports community-based approaches that:

    • Expand existing mobile healthcare services in rural communities
    • Improve connections to social supports and community resources

    Organizations may apply under one or both priority areas described in this RFP.

    Background

    Washington County encompasses urban, suburban, and rural communities. Rural areas across the county are characterized by small towns, farms, and incorporated and unincorporated communities. Residents in these communities may experience limited access to healthcare services, transportation, and other supports that contribute to overall health and wellbeing.

    WCPH works alongside community partners to support the health and wellbeing of residents throughout the county, including rural communities. Several rural communities in Washington County are designated as Medically Underserved Populations (MUPs) by the U.S. Health Resources and Services Administration (HRSA), a designation used to identify geographic areas experiencing shortages of primary care services.

      The organization demonstrates relevant experience serving rural communities or communities with similar needs, providing comparable services, and successfully implementing projects of similar scope. The proposal reflects sufficient organizational capacity to carry out the proposed activities.

      Applicants will be scored from 0 – 5 points based on the extent to which the proposal demonstrates the following:

      • 0 – 1 point: Provides limited information regarding the organization’s mission and background, populations and geographic areas served, relevant experience, current programs or services, and organizational capacity to implement the proposed project.
      • 2 – 3 points: Provides sufficient information describing the organization’s background, experience, programs, and capacity, but some information lacks detail or only partially demonstrates experience implementing comparable work or serving similar communities.
      • 4 – 5 points: Clearly and thoroughly describes the organization’s mission and background, populations and geographic areas served, relevant experience, current programs or services, and organizational capacity to successfully implement the proposed project.
    • Project Implementation (40 pts)

       

      The proposal presents a clear, feasible, and community-responsive approach for implementing the proposed project, including proposed activities, the rural Washington County communities to be served, partnerships or collaborations that support implementation, and a realistic timeline for completing project activities.

      Applicants will be scored from 0 – 5 points based on the extent to which the proposal demonstrates the following:

      • 0 – 1 point: Provides limited or unclear information regarding proposed activities, communities to be served, partnerships or collaborations, implementation timeline, or key milestones.
      • 2 – 3 points: Describes the proposed activities, communities to be served, partnerships or collaborations, and implementation timeline, but some elements lack detail, feasibility, or a clear connection to the communities to be served.
      • 4 – 5 points: Presents a clear and feasible implementation approach that includes well-defined activities, identified rural communities to be served, appropriate partnerships or collaborations, realistic timelines and key milestones, and demonstrates responsiveness to the communities being served.
    • Project Impact (25 pts)

       

      The proposal clearly describes the anticipated outcomes of the proposed project, how the proposed project is expected to improve access to healthcare services and/or social supports in rural Washington County communities, and how success will be assessed.

      Applicants will be scored from 0 – 5 points based on the extent to which the proposal demonstrates the following:

      • 0 – 1 point: Provides limited information regarding anticipated outcomes, how the proposed project is expected to improve access to healthcare services and/or social supports, or how success will be assessed.
      • 2 – 3 points: Describes anticipated outcomes, expected improvements in access to healthcare services and/or social supports, and general approaches for assessing success, but the information provided lacks detail or only partially explains the expected impact of the proposed project.
      • 4 – 5 points: Clearly describes anticipated outcomes, explains how the proposed project is expected to improve access to healthcare and/or social supports, and presents realistic approaches for assessing whether the proposed project is achieving its intended results.
    • Budget Narrative (10 pts)

       

      The budget is reasonable, clearly connected to proposed activities, and demonstrates appropriate use of funds.

      Applicants will be scored from 0 – 5 points based on the extent to which the proposal demonstrates the following:

      • 0 – 1 point: Provides an incomplete, unclear, or insufficient explanation of the proposed budget and intended use of funds.
      • 2 – 3 points: Presents a generally reasonable budget and budget narrative, though some budget categories or justifications require additional explanation.
      • 4 – 5 points: Presents a clear, reasonable, and well-justified budget narrative that demonstrates how each major budget category supports the proposed project activities and allowable use of funds.
    • Interviews (If Necessary) (1 pts)

      Should the County determine that interviews are necessary, proposal evaluation scores will be used to determine who goes to interview.  There will be a new scoring rubric specifically for the interview process.  The interview process will determine the successful proposer.​

      The proposal shall be prepared succinctly, providing a straight forward, concise description of the proposer’s ability to meet the requirements of the RFP. There should be no unnecessary attachments or exhibits.

      Proposal should contain no more than eight (8) 8.5" x 11", pages of written material (excluding biographies and brochures which may be included in an appendix) describing the ability of the proposer to perform the work described. The minimum font shall be twelve (12) point single spaced and the minimum margins shall be one (1) inch on all sides. Any pages over the page limit will not be read or scored.
