TRWD is hereby inviting Statements of Qualifications (SOQs) from qualified firms to provide services for Construction Materials Testing and Inspection Services.Do not provide any fee estimates, pricing or billing rate information as part of the SOQ submittal.

The Scope of Work is not comprehensive. Rather, it represents what TRWD anticipates will be needed for this project. Prospective vendors may recommend improvements to the scope in the project approach section of their SOQ (Statements of Qualifications). The selected firm will prepare a comprehensive scope that will be negotiated with TRWD.

  Complete the form, sign and date where indicate. We are required to have a completed copy of the CIQ form to be signed and dated even if there is(are) no conflict(s). Form is not considered complete unless signed and dated.

• Form 1295 – Certificate of Interested Parties (required) —

  Complete form online at https://www.ethics.state.tx.us/filinginfo/1295/ using the following assigned identification number and description for this Project:

  Upload completed form to include certificate number and signature.

  ***FYI – The “Affidavit” section has been replaced and is now the “Unsworn Declaration”.  This Section will require the date of birth and address of the authorized representative signing the form.  The change in law applies to contracts entered into, renewed, or amended on or after January 1, 2018.
