This includes delivery of items and the catagories are as follows: Catagory 1 ability place orders for 200 each per style Catadory 2 ability to place order per style for all items How to use this contract: 1. View the Ordering and Pricing and determine which custom uniform items and alteration services are needed. 2. Contact the vendor with requests for quotes, uniform and alteration questions, or to place an order. All vendor contact information is located in the Contact Information. 3. Orders can be made by telephone, email, web-based through a purchase order provided to the contractor. It should include an itemization of the uniforms being ordered, any alteration services being provided, and contain the authorized signatures of both parties. 4. Be sure to reference Contract #05719 when you contact the vendor so you get pricing consistent with the Statewide Contract. 5.
