    REQUEST FOR QUOTES DESCRIPTION: This project is for design work of a residential bathroom renovation. Design to include all demo work, and construction of a new bathroom with waterproof tiled floor, walk in shower with 4” curb and solid surface surround, and standard toilet and vanity. The Design Vendor provide all labor and materials necessary to complete a fully comprehensive drawing design and specification generation of the services required in a professional manner. Refer to the attached specifications (if applicable) for the following OPWDD Requirements of the Project Scope of Work. Environmental Surveying has been completed, with ACM Identified. Demo plans must include a proper abatement phase compliant with ICR56. SCOPE OF WORK includes, but is not necessarily limited to the following: A. Design Services: 1. Conduct an initial site visit to understand the goal of the design. 2. Reference attached specific OPWDD BATHROOM DESIGN STANDARDS for guidance. 3. Drawing Designs and Specifications are to be in accordance with: a. 14 NYCRR 635, Subpart 635-7 b. 19 NYCRR 1201 and 1204. c. OPWDD Specifications, d. NFPA (NFPA 101 - Life Safety Code) e. NYS Uniform Building Code, f. NYS Environmental and Energy Code Regulation Compliance. g. NFPA 101A – Guide on Alternative Approaches to Life Safety. 4. Design shall include Product Specification Data. a. Construction details, material descriptions, dimensions, profiles, finishes, and applicable schematics, and cut sheets. 5. All Design and Specification documents shall be provided on Adobe (.pdf), CAD Files should be provided to OPWDD for future use and records. 6. Document Submissions to be provided to OPWDD Point of Contact (POC) at routine Intervals: a. 60% - Include rough construction estimates based on prevailing wage. b. 90% Completion c. 100% Completion (Issued for Permitting- signed and stamped) 8. Internal Construction Permitting to be completed by OPWDD. Design Vendor shall provide (2) sets of 24” x36” blueprints upon completion, mailed to POC. 9. A Probable Construction Cost Estimate shall be provided along with the 100% drawings, based on current pricing, materials, supplies, and prevailing wage table for the appropriate county. Estimate to be completed on OPWDD provided form. 10. Design Proposal shall include a time schedule for the Design Services based on current workload within the Design Firm. 11. Generalized Statements required by OPWDD POC for protection of Service Recipients and Operations, on a case-by-case basis. B. Future Construction Administration Services are to include: 1. Review and Respond to Requests for Information (RFI) from site visits with prospective contractors. 2. Review submittal documents and shop drawings from contractors. 3. Provide comments, and recommendations for components (as-equal products) based on submittals and shop drawings. 4. Provide at least (3) site reviews. a. Initial site review of existing conditions for design basics. b. Review of project during construction phase for field observation report. c. Upon project completion for design/code compliance. 5. Upon completion of final site review, a punch-list shall be generated for the contractors to complete.
