    Q (LFG Information Request ): Please provide an all-eligible census along with elections for voluntary employees

    A: I will add this to the "Attachments" section of this RFP and issue an addendum shortly. Thanks!


    Q (Combined Certifications and CIQ link errors): I'm receiving errors when trying to access the links provided for the Combined Certifications and CIQ documents provided on page 13 of the "Employee_Benefits.pdf" document.

    A: These links are meant to work when you're online, in the "Vendor Questionnaire" section. You can also go to the top right corner of the screen in OpenGov. Select EXPORT -> DOWNLOAD FULL DOCUMENT -> FULL PACKET WITH ATTACHMENTS. Let me know if you've tried that and are still having difficulties.


    Q (Legal Plan): While not specifically requested, would you like us to provide a voluntary legal plan option for consideration? These plans are offered at no cost to the employer and can enhance your benefits package by providing employees with affordable access to legal service

    A: Not at this time.


    Q (Large Claims): Please provide the last three years of large claims data.

    A: These are already included in the Medical subproject of this RFP under “Total Paid Claims” from 2023-2026.


    Q (Census): Please provide an employee census with five-digit zip codes.

    A: Please see the census document in the "Attachment" section of this RFP. The information that you request is in Column F.


    Q (Narrow Network): Is the City looking into the possibility of utilizing a narrow network?

    A: The City currently offers an EPO and HSA and we would like to model as close as possible to current.


    Q (Claim Experience Summaries): Please provide the last three years of claim experience summaries.

    A: These are attached in the "Medical" sub-project. Please see attachment G.


    Q (SBCs): Please provide current SBCs.

    A: I have just released an addendum with these documents.


    Q (Submittal Form): The RFP mentions: Proposals submitted without the completed submittal form for the appropriate product may be disqualified as a candidate for that product." Where is the submittal form located?

    A: These forms are in the "Vendor Questionnaire" section of the main RFP and each of the associated projects. It's an excel spreadsheet that is attached to one of the questions.


    Q (HSA and FSA): Can you please let us know what the Assets Under Management are for the HSA and how much of that is in cash deposits and how much in investments? Also, the contributions on the census for the HSA and FSA, what frequency are those contribution amounts? Monthly, bi-weekly, semi-monthly?

    A: Regarding the Assets Under Management - that information is not available. Regarding the frequency of the contributions - contributions align with our payroll (bi-weekly).


    Q (BAA): The Questionnaire tab within the Dental Vendor Response Document asks for carriers to agree to the terms and conditions in the BAA. However, there was no BAA provided. Please provide if you'd like us to review.

    A: No BAA will be provided. Please disregard this requirement.


    Q (Medical Claims Data ): Please provide at the minimum 12 consecutive months of claims experience, including large claims for the same period. This is needed for our review and analysis.

    A: This data was included in the original posting. Please refer to the "Attachments" section of the Medical project.


    Q (No subject): Life Underwriting: 1. The Life/AD&D Vendor Response workbook shows “current” basic life/AD&D rates of $0.140/$0.025, but the City’s current rates are $0.125/$0.025. Please correct this error. 2. The Life/AD&D Vendor Response workbook, “Basic Life AD&D Rates” tab says to “include Flat 10% commissions” with our basic life/AD&D quote. Current rates are net of commissions. Please clarify whether commissions are requested on the life/AD&D quote. 3. The Life/AD&D Vendor Response workbook, “Plan Design Voluntary Life” tab indicates that the current voluntary life plan has a minimum participation requirement. This is not true, the current plan has no minimum participation requirement. Please correct this error.

    A: The Basic Life rate is currently $0.125/$0.025, please note that this is an update from the workbook that reflects $0.140/$0.025. All Life/AD&D responses should be quoted net of commissions. There is currently no minimum participation.


    Q (No subject): Census Clarifications: 1. The provided census does not indicate basic life volumes for each employee on the “Basic and Voluntary Life & AD&D”. There are several duplicate employees, and when you filter out these duplicates, there are only around 130 unique employees, but the RFP document says there are 267 full time employees of the City. Please provide a census that includes all 267 of these employees along with their current salaries and/or basic life/AD&D amounts.

    A: I am going to add a file called "Updated Basic Life Census - 04.01.2026" to the Attachments section in the main RFP. Please watch for the addendum. This is a census with all full time employees.


    Q (EAP ): Are there any areas for improvement with the current EAP offering?

    A: The City is conducting an RFP to ensure that employee benefits are competitively priced and aligned with employees’ needs. All proposals will be thoroughly evaluated, and the City will select the option that best meets these criteria

    Q (Vision Provider File): Can you please provide an updated Vision provider utilization file to complete a Vision disruption analysis? The file that was included does not include zip codes. We will need full address, as well as TIN/NPI to run a disruption analysis.

    A: The incumbent vendor was not able to supply a report with zip codes or Tax IDs at this time. NPIs are included within the document titled “Top 100 Provider Data Report by NPI.
