Through this Request for Proposal (RFP) the Agency of Human Services, Department of Vermont Health Access is seeking to strengthen Blueprint’s capacity to advance high-performing primary care practices that deliver coordinated, patient-centered, and population-focused care. This RFP details the proposal submission and evaluation processes, the required experience of the bidder, the scope of work to be performed, and the resulting contract deliverables. The State may enter into a contract to have the selected vendor(s) perform all or part of the tasks identified in Attachment A of this RFP.
