Through this Request for Proposal (RFP) the Agency of Human Services, Department of Vermont Health Access Vermont has received a Rural Health Transformation (RHT) grant of $195 million in Year One of a five-year opportunity. The purpose of the grant is to build stronger rural health networks, improve technology and shared operations, strengthen the rural health workforce and ultimately ensure that Vermonters receive the right care at the right time for an affordable cost. Through this Request for Proposal (RFP) the Agency of Human Services, Department of Vermont Health Access (hereinafter the “State”) is seeking to provide Advanced Primary Care Learning Collaboratives to support primary care practices in implementing evidence-based approaches to improve clinical outcomes and strengthen team-based care This RFP details the proposal submission and evaluation processes, the required experience of the bidder, the scope of work to be performed, and the resulting contract deliverables. The State may enter into a contract to have the selected vendor(s) perform all or part of the tasks identified in the Desired Services described in Section 2 of this RFP.
