    City and County of San Francisco Human Services Agency (HSA) Funding Opportunity Request for Proposals (RFP) #1225 Older Relative Caregiver Program The San Francisco Human Services Agency (SFHSA) announces its intent to seek proposals from nonprofit organizations interested in providing older relative family caregiver support services for the Department of Disability and Aging Services (DAS). Older relative family support services are an array of services designed to support caregivers in maintaining quality home care and to improve both caregivers’ and care recipients’ health and well-being. The purpose of this grant is to assist San Francisco residents 55 and older who are the grandparent, step-grandparent, or other older relative of a child (under 18 years of age) or individual of any age with a disability whom they are related to by blood, marriage, or adoption, whom they live with, and who are identified as the primary caregiver of the child or individual with a disability through a legal or informal arrangement. Qualified proposer will provide services to family caregivers caring for children 18 years of age or younger, or caring for an individual with a disability. Services include, but are not limited to, community education for current and potential caregivers; outreach activities and information and assistance to caregivers; support services, including caregiver assessment, caregiver counseling, caregiver support groups, caregiver training, and case management; respite service, including both in-home and out-of-home respite service; and supplemental services, such as legal assistance and provision of assistive devices.
