City and County of San Francisco Human Services Agency (HSA) Funding Opportunity Request for Proposals (RFP) #1227 Temporary Respite Caregiver Support Program The San Francisco Human Services Agency (SFHSA) announces its intent to seek proposals from nonprofit organizations interested in providing respite care to unpaid family members, family of choice, and/or individuals (e.g., friends) who are caregivers to older adults and adults with disabilities as defined in this RFP. Applicants may elect to apply for respite care as a lead agency with collaborative partnership(s) in place. Applicants must clearly identify roles of all partnerships if named in the proposal. Proposals must be for respite care only. Other types of caregiver support services such as community education, caregiver support, service information, etc., are not eligible for funding through this RFP, as these services are available through existing Family Caregiver Support Services programs. This is an existing, fully enrolled program. All proposers must agree to work closely with DAS in the event of a grantee change in order to ensure a smooth programmatic transition and a minimum of service and client disruption.
