    Description

    The Napa County Health and Human Services Agency – Self Sufficiency Services (SSS) is requesting qualified proposals from experienced providers for the California Work Opportunity and Responsibility to Kids (CalWORKs) Home Visiting Program (HVP). The purpose of HVP is to provide voluntary, high-quality, evidence-based, and culturally competent home visiting services to support the positive health, development, and well-being outcomes for pregnant and parenting women, families, and infants born into poverty.  HVP aims to expand CalWORKs participant’s educational, economic, and financial capability opportunities to improve the likelihood that they will exit poverty.

     

    Providers submitting qualified proposals must have a home visiting model that has demonstrated evidence of effectiveness.  Evidence-based home visiting models that are not evaluated by Home Visiting Evidence of Effectiveness (HomVEE) must submit a copy of the model’s evaluation.

    Evaluation Criteria

    • Executive Summary and Approach (40 pts)
        1. Explain Scope of Work and specify how the services in the proposal will meet or exceed the requirements of the County.
        2. Specify proposer’s ability to provide the services
        3. Explain any special resources, procedures, or approaches that make the services of proposer particularly advantageous to the County.
        4. Identify providers ability to sustain home visiting services.
        5. Identify frequency and duration that provider will provide services.
    • Qualifications, Experience, and Knowledge (20 pts)
        1. Specify years or experience providing home visiting services.
        2. Home Visiting Model meets evidence-based model based on HomVee or evidence of evaluation.
        3. Describe the knowledge, training and expertise that staff must meet the minimum qualifications and training requirements.
        4. List any relevant certifications of all staff.
        5. Briefly describe your knowledge, experience, and ability to administer a program that satisfies the scope of service described in your proposal.  Include types of services you have previously provided and the length of time you have provided them.
    • Resources and Collaboration (15 pts)
        1. Identify resources and connections provider must provide appropriate referrals and needs to participants.
        2. Identify any multi-disciplinary group participation that focuses on home visiting.
    • Budget and Budget Narrative for Activities (25 pts)
        1. Detail the cost of your proposed activities outlined in the Scope of Work, including proposed number to be served.

      The County reserves the right to contact references not provided in the submittals.

    • You are required to disclose any alleged significant prior or ongoing contract failures, any civil or criminal litigation or investigation pending which involves the Proposer or a verification. (required)

      Failure to comply with the terms of this provision may disqualify any proposal. The County of Napa reserves the right to reject any proposal based upon the Proposer’s prior history with the County of Napa or with any other party, which documents, without limitation, unsatisfactory performance, significant failures to meet contract milestones or other contractual failures.

      PSA: Respondents should clearly note in their proposals whether they take exception to any of the PSA requirements and should include a detailed explanation of the reason for the exception and a counterproposal or alternative suggestion(s) for the County’s review. However, after review of any respondent’s exceptions, the County reserves the right to reject any and all exceptions taken to the County’s PSA.

      Insurance: Respondents who are unable to meet all the County’s insurance requirements may submit with their proposal an alternative plan for obtaining insurance that will adequately mitigate the risks associated with providing the services. Any alternative insurance coverage request is subject to review and approval by County Counsel and the County’s Risk Management Officer.

      Failure to meet the County’s insurance requirements (as determined by County Counsel and County Risk Management) may be sufficient reason for disqualification from the selection process.
