    Description

    Project Goals: The State of Delaware Department of Health and Social Services, Division of Public Health is requesting proposals to develop and conduct appropriate statewide activities that will advocate and encourage improved health outcomes. Successful applications will propose evidence, practice, or research-based strategies that support expanding inclusivity and improve health equity. These strategies can include: • Implementing recommendations in The Impact of Diabetes in Delaware, 2025 biannual report, The Burden of Chronic Disease in Delaware 2024, and the Delaware Cancer Consortium (DCC) state cancer goals and objectives found in the Five-Year Cancer Plan 2022-2026. • Facilitating the vision to make Delaware the healthiest state in the nation as outlined in the Delaware State Health Improvement Plan (SHIP) 2025-2028.The RFP awardee will be responsible for demonstrating a comprehensive methodology improving the health of Delawareans through prevention and management of prediabetes and diabetes. The most successful proposals will include dynamic, innovative and evidence-based interventions that also can assimilate to the needs of the state’s management of chronic disease in general and diabetes specifically. The successful bidder will provide expertise in the planning, development, and implementation of health promotion activities.The purpose of this RFP is to procure professional services for specific activities that align with component A of the Center for Disease Control and Prevention (CDC) cooperative agreement with Delaware, “A Strategic Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes,” as well as support the DPH’s response to statewide strategies aimed at mitigating the impact of chronic disease risk factors affiliated with diabetes, hypertension, cardiovascular disease, cancer, obesity, asthma, and other chronic conditions.
