The most successful proposals will include dynamic, innovative and evidence-based interventions that also can assimilate to the needs of the state’s management of chronic disease in general and diabetes specifically. The successful bidder will provide expertise in the planning, development, and implementation of health promotion activities.The purpose of this RFP is to procure professional services for specific activities that align with component A of the Center for Disease Control and Prevention (CDC) cooperative agreement with Delaware, “A Strategic Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes,” as well as support the DPH’s response to statewide strategies aimed at mitigating the impact of chronic disease risk factors affiliated with diabetes, hypertension, cardiovascular disease, cancer, obesity, asthma, and other chronic conditions.
