The successful bidder will provide expertise in the planning, development, and implementation of healthy living and health promotion activities.The purpose of this RFP is to procure professional services for specific activities that align with strategies in the Center for Disease Control and Prevention (CDC) cooperative agreement with Delaware, “The National Cardiovascular Health Program,” as well as support the DPH’s response to statewide strategies aimed at mitigating the impact of chronic disease risk factors affiliated with: • hypertension, • cardiovascular disease, • diabetes, • cancer, • obesity, • asthma, and • other chronic conditions.Please refer to Appendix B for full details
