    The Delaware Food is Medicine (FIM) initiative aims to transform chronic disease prevention and management in rural Kent and Sussex counties by integrating nutrition into healthcare delivery. Aligned with the strategic goal of “Make Rural America Healthy Again,” This initiative addresses the high prevalence of diet-sensitive conditions, such as diabetes, hypertension, and chronic kidney disease, while reducing healthcare costs and promoting health equity.Through a coordinated systems-based approach, the program establishes sustainable infrastructure for medically tailored nutrition interventions, builds workforce capacity, and strengthens community partnerships. By targeting vital conditions, including nutrition security, access to care, and economic stability, the initiative ensures that rural populations receive equitable, evidence-based support to improve dietary health, manage chronic diseases, and achieve long-term well-being.The following five goals define the scope of this initiative: • outlining strategies to expand access, • improve health outcomes, • advance equity, • build workforce and partnerships, and • establish replicable, sustainable FIM infrastructure.Please refer to Appendix B for additional details/requirements for the Scope of Services.
