Project GoalsThe primary goals of this project are to: 1. Complete Annual Progress Narrative for NCCCP and submit into the CDC’s designated platforme. Complete NBCCEDP Evaluation Report and submit it into the CDC’s designated platform f. Complete NBCCEDP Annual Progress Report, including service delivery components and submit it to the CDC’s designated platform g. Provide program summary reports, as requested, to the Delaware Cancer Consortium or DPH leadership. • Ensure that all required submissions to CDC are accurate, complete, and submitted on time.Foster Coordination Across Program ComponentsWork with all cancer grant programs to ensure alignment and consistency between NCCCP and NBCCEDP deliverables and data. May collaborate with NPCR staff for information gathering and document preparation.Provide consultation for program success stories and may draft success stories (note: success stories final version will be written by program directors).Participate in Meetings and Technical AssistanceAttend CDC meetings, DCC meetings, and stakeholder meetings as assigned or invited by the Bureau Chief, Comprehensive Cancer Control Program Administrator, or Screening for Life Program Administrator.Enhance Program Evaluation InfrastructureSupport the establishment of uniform evaluation practices and assist with miscellaneous data analysis requests.Assist in conducting audits of Delaware Cancer Registry data to maintain data quality and reporting accuracy.Assist as requested in creating and distributing surveys to clients, participating health care providers, and other stakeholders to determine program efficiency and effectivenessScope of Services Please refer to Appendix B for additional details/requirements for the Scope of Work.
