Project GoalsThe primary goals of this project are to: 1. Assist clients with Screening for Life and Health Care Connection applications - Provide individualized, face-to-face in-person assistance to clients to complete the full application process for SFL and HCC programs. - Ensure all applications are 100% completed, including all required supporting documents and signatures, before submitting them to the SFL/HCC Programs.Promote early detection through cancer screening outreach- Educate clients on the importance of early detection and available preventive screening services. - Increase the number of eligible clients who complete recommended cancer screenings. - Ensure all clients who are successfully enrolled in the SFL program are linked to a patient navigator to schedule age-appropriate cancer screenings within 30 days of enrollment. - Ensure all female clients aged 35 to 64 are offered WISEWOMAN services during their breast or cervical cancer screening. This requires completion of the WISEWOMAN baseline client intake form and update of the client’s WISEWOMAN data tab in the SFL database, and case note in the SFL database.Enhance care coordination and reduce barriers- Track client applications through the enrollment verification process and coordinate with patient navigators and project managers to schedule screening appointments and WISEWOMAN integrated office visits. - Identify and address barriers to screening (e.g., transportation, documentation, language).Maintain accurate and timely data tracking- Complete daily data entry and client updates in the SFL database and any other data collection tool utilized by the SFL program (e.g. Smartsheets) to support program reporting and accountability.Support statewide cancer prevention initiatives- Strengthen partnerships between FQHCs, community centers, and public health programs to promote cancer screening services, including the WISEWOMAN program. - Contribute to statewide goals for increased screening uptake and improved health outcomes.Services under this RFP may be awarded to multiple vendorsScope of Services Please refer to Appendix B for additional details/requirements for the Scope of Work.
