      1. Describe the specific strategies and activities to be used to achieve the stated objectives, expectations, and desired outcome in the RFP, including your agency’s plan for offering services outside of standard business hours.
      2. Describe specific strategies and activities used to engage families to ensure their participation in face-to-face contact in their homes.
      3. Describe your staffing model, including where staff are geographically located and how travel time is managed. Explain how your proposed pricing accounts for routine travel and how you ensure efficient service delivery while minimizing non-client-facing billable time.
    • Experience and Qualifications (20 pts)
      1. Include your agency’s mission statement and discuss how this proposed program aligns with that mission.
      2. Describe the experience and qualification of your agency to provide programs that are welcoming to persons of all backgrounds and cultures including any explicit plans your agency is undertaking to improve in this area.

      4. Describe knowledge and understanding of WI DCF Safety Standards.
      5. Describe experience working with County Child Protective Services organizations. Please explain how your organization maintains/will maintain excellent communication standards with Child Protective Services (CPS) staff and why this is important for the services within this RFP. 
      6. Please describe your organization’s staff retention and workforce stability for the past three years as it relates to this service area. Include annual turnover rates for key positions, vacancy rates, average length of staff tenure, and any challenges in recruitment or retention. Describe the strategies your organization uses to recruit, retain, and support qualified staff, particularly in high-demand or hard-to-fill roles, and explain how staffing stability is maintained to ensure continuity of services for clients.
      7. Submit 2 - 3 letters of reference. References should be specific to the service offered. References should be from agencies you have done business with or those with whom you have collaborated. [Reference checks may be conducted with these agencies and others in the community.] 
    • Quality Improvement (15 pts)
      1. Describe any programmatic or administrative improvements that have improved your agency’s ability to deliver services. Also describe any ongoing quality assurance procedures and practices your agency has in place.
      2. Describe how program data is or will be used to drive quality improvement efforts, including the measurement of and performance on client outcomes. 
    • Cost (20 pts)

      Points for cost will be awarded using a relative formula. The proposal with the lowest cost will receive the maximum number of points available for this section. All other proposals will receive a proportion of the cost points according to the following calculation:

      In plain language: the lowest-cost proposal earns all available points. Other proposals earn fewer points based on how their cost compares to the lowest. For example, if your proposal costs twice as much as the lowest proposal, you would receive about half of the cost points.

    • Local Vendor Preference (5 pts)

      Per Dane County ordinance, a local Dane County vendor automatically receives five (5) points toward the evaluation score.

      Vendors located within the counties adjacent to Dane County (Columbia, Dodge, Green, Iowa, Jefferson, Rock, or Sauk) automatically receive two (2) points toward the evaluation score.

      Locally Operated Vendor means a supplier or provider of equipment, materials, supplies, or services which has an established place of business within Dane County and whose business is registered and authorized to do business in the State of Wisconsin. An established place of business means a physical office, plant, or other facility. A post office box address does not qualify a vendor as a Locally Operated Vendor. Dane County Ordinance 25.04(5)

    Q (Vendor Questions #5 Costs): There is a confusing reference to calculating the # of beds in the pricing proposal. Is this an error?

    A: It was an error, thank you. Please see addendum #1.


    Q (Scope of Services 3.7 Funding Structure): Is travel time to/from service location billable?

    A: Please see section 3.7: The unit of service for this purchase of service engagement equals: one (1) hour of direct service provided to a client by qualified staff. Billable time is limited to direct client contact, whether delivered in person, by telephone, or by video. Time spent on documentation, service planning, care coordination, transportation, staff meetings, supervision, and training is not billable. Home visits conducted when the client is not present may be billed in increments of 0.25 units.
