    Issuing Procurement Unit: State of Utah Division of Purchasing Conducting Procurement Unit: Department of Veterans and Military Affairs (UDVMA)The purpose of this RFP is to enter into a contract to provide: Veterans Homes Management and Operation.It is anticipated that this RFP will result in a single contract award to the responsive and responsible vendor with the highest score justified by the procurement code .MANDATORY Site Visit:Offeror’s MUST attend MANDATORY Site Visit. Site Visit will begin promptly on the 13th of April, 2026 at 10:00 AM local time at the William E Christoffersen Veterans Home (WECVH) located at 700 Foothill Dr. Salt Lake City, UT 84113. We will meet in the building lobby entrance. We anticipate this initial location to last approximately 90 minutes. We will then meet at the George E. Wahlen Ogden Veterans Home (GEWOVH) at 1:30 PM to conduct this final site visit on the same day. ALL Offeror’s MUST be in attendance for the entire site visits. ANY Offeror that is not present and on time for each stated location site visit will be excluded from this solicitation.The methodology or formula that will be used to determine this contract award is:Stage 1: Initial Review In the initial phase of the evaluation process, the conducting procurement unit will review all proposals timely received. Non-responsive proposals not conforming to RFP requirements or unable to meet the mandatory minimum requirements will be eliminated from further consideration.Stage 2: Technical Proposal Evaluation Responsive proposals will then be evaluated by an evaluation committee appointed by the conducting procurement unit against the proposal evaluation criteria noted in the Technical Criteria prerequisite of this RFP. Proposals will be evaluated against the evaluation criteria as follows:Experience - 100 points possible Quality Measures – 100 points possible Staffing - 100 points possible Health Inspections – 100 points possible Food and Dining – 75 points possible Quality of Life - 75 points possible Honoring Veterans - 75 points possible Replacement Facility Transition - 75 points possibleTotal Possible Points - 700 PointsOfferors that achieve a minimum score threshold of 560 (80% of total available points) in the technical proposal stage will proceed to the Final Stage: Cost Proposal EvaluationThe Offeror with the lowest total cost will receive the maximum points of 100. Points assigned to each Offeror’s cost proposal will be based on the lowest proposal price. The Offeror with the lowest total cost will receive 100 or 100% of the total cost points. All other Offerors will receive a portion of the total cost points based on the percentage higher their total cost is than the total lowest cost. The formula to compute the points is: Cost Points x (Lowest Proposed Price/Proposed Price).Pursuant to Utah Code Annotated (UCA) § 63G-6a-707(7), the cost proposal will be evaluated independently from the technical proposal, and as such, must be submitted separately from the technical proposal. Offerors must not include costs or pricing data in their responses to the Mandatory Minimum Requirements and the Technical Response.Offerors with a score of less than the minimum required technical points will be deemed non-responsive and ineligible for further consideration. The proposal that receives the highest total points from the combined technical and cost scores will be awarded a contract. The evaluation score sheet has been attached to this RFP. The attached evaluation score sheet states the relative weight that will be given to each evaluation criteria.Length of Contract: 5 Years with One (1) optional 3 Year Renewal if exercised by UDVMA.Vendor must guarantee its pricing for the term of the contract.
