The Minnesota Department of Veterans Affairs (“MDVA” or “State”) is seeking proposals from qualified organizations to coordinate and deliver comprehensive COVID-19 testing and vaccination services for facility staff at all eight Minnesota Veterans Homes (“MVH” or “Homes”). The selected Vendor will ideally provide services to all eight Minnesota Veterans Homes and have an established testing program that includes all components from ordering provider, swabbing team, lab services, and communication of results. The MDVA operates continuously and is responsible for the health and well-being of more than 800 residents across its eight Minnesota Veterans Homes. Each Home is licensed by the Minnesota Department of Health (“MDH”), and all are included within the scope of this solicitation as listed below: • MVH – Minneapolis, 5101 Minnehaha Avenue, Minneapolis, MN 55417 o Approximate staff numbers: 600 • MVH – Hastings, 1200 18th Street E, Hastings, MN 55033 o Approximate staff numbers: 89 • MVH – Luverne, 1300 N Kniss Avenue, Luverne, MN 56156 o Approximate staff numbers: 155 • MVH – Fergus Falls, 1821 N Park Street, Fergus Falls, MN 56537 o Approximate staff numbers: 193 • MVH – Silver Bay, 56 Outer Drive, Silver Bay, MN 55614 o Approximate staff numbers: 128 • MVH – Montevideo, 2190 William Avenue, Montevideo, MN 56265 o Approximate staff numbers: 125 • MVH – Bemidji, 920 Anne Street NW, Bemidji, MN 56601 o Approximate staff numbers: 120 • MVH – Preston, 1864 First Minnesota, Preston, MN 55965 o Approximate staff numbers: 99 The MDVA prefers to award a single contract to a vendor capable of delivering the full scope of COVID-19 testing services across all eight Homes.
