Successful contractor shall be required to complete a contract and all related documents, including but not limited to Vendor Responsibility, NYS Finance Law 139 and NYS Tax Law Section 5-a. In addition, the contractor shall be required to submit valid and current NYS Workers' Compensation and Disability Forms.
