Nutrition Consultant Services / Scope of WorkThe type and frequency of consultation services shall be determined through mutual agreement between the Head Start/Early Head Start Director (or designee) and the Nutrition Consultant. Consultation activities, nutritional assessments, and related documentation may be maintained in the agency’s designated reporting system, including ChildPlus.Nutrition consultation services may include, but are not limited to:Meeting and collaborating with families, as needed, to obtain additional nutrition information and support planning for care at school.Conducting home visits, when necessary.Consulting with community medical providers to obtain additional nutrition information and/or recommendations regarding the child’s care at school.Providing guidance and technical consultation to staff regarding individual child nutrition concerns, including:-Development of Individualized Health and Nutrition Plans (IHNPs)-Development of 504 Accommodation Plans-Review of plans with staff-Food allergies-Dietary restrictions-Food substitutions-Health conditions related to nutrition-Appropriate accommodations for children with identified nutrition needsParticipating in Multidisciplinary Team (MDT) meetings, as applicable to the child’s individual needs.Providing consultation regarding nutrition services and related concerns to program staff and families, as requested.Collaborating with staff involved in the child’s care, as appropriate to support nutrition-related accommodations and services.Reviewing enrollment forms and applicable documentation related to identifying children’s nutrition and health needs, including but not limited to:-Physical examinations-Growth assessments-Dental examinations-IEPs-Medical statements-Medical orders-Medication labelsLead and hemoglobin resultsProviding consultation regarding menu accommodations for children with dietary needs and/or food accommodations in accordance with CACFP requirements.Reviewing menus, as applicable, to support compliance with CACFP guidelines.Communicating identified family nutrition goals with appropriate program staff following completion of nutrition assessments.Nutrition Milestones and TimelinesApplicable ChildPlus reports and related documentation may be reviewed in accordance with program compliance requirements and contracted services.Applicable reports may include:3065 / 3035 – Food accommodations and RD Referral ReportsDocumentation related to dietary accommodations and nutrition servicesApplicable follow-up activities and related documentation may be maintained in ChildPlus in accordance with agency procedures and regulatory requirements.Written ReportsThe Nutrition Consultant may provide periodic documentation related to contracted services, which may include:-Service logs-Accountability or consultation summaries-Documentation of consultation activities-Invoices for services renderedDocumentation submitted for invoicing purposes should align with services provided during the applicable service period.Monitoring of Performance and Quality of ServicesThe agency may review submitted documentation, consultation summaries, and related deliverables to ensure contracted services align with program requirements and applicable Pre-Bid Meeting Information: No Online Q&A: Yes Location Information: Department: Procurement Department Address: 401 N.
