Establish an account with the awarded contractor of your choice by contacting the noted representative, if not already active. Comprehensive pricing is only available by logging in to the contractor's online portal. 3. Login to the contractor's online portal to access available products at approved pricing and place orders. Reference State Contract No. 19222 where possible on your ordering documents. To complete a return, the following information will be required: • Current DEA License • Current State Board of Pharmacy License • Copy of Wholesaler Invoice (No More Than 90 Days Old) • MMCAP Member ID • Facility Contact Information • Physical Address • Billing Address 4.
