    Through joint efforts of the MDH Prevention and Health Promotion Administration (PHPA), Behavioral Health Administration (BHA), and the Health Services Cost Review Commission (HSCRC), this RFA is to award funding under the Maryland Rural Health Transformation Program (RHTP). Focused on rural Maryland, the goals of these Transformation Funds include: Expanding primary care, specialty care, and behavioral health care to meet current demand and build long-term capacity to improve health outcomes; Promoting efficient and effective management of chronic somatic and behavioral health conditions, including by empowering residents to manage their conditions; Leveraging technology to bring healthcare directly to rural residents; Empowering residents to manage somatic and behavioral chronic conditions; and Helping providers adopt innovative, value-based care models.
