E-rate Solicitation Bid Title: PiuteSD 2026 E-Rate Cat 2 Utah Public Procurement Place Bid Number: PiuteSD2026eratecat2 Closing Date: Noon, March 31, 2026. 1. Bidders must bid the brand and model listed or a "functionally equivalent" product. Piute School District reserves the right to be the determining entity of what is functionally equivalent. Product bid must be compatible with existing Piute School District equipment. Award will be made to the Bidder with the highest score based on Piute School District's Bid Evaluation Matrix (available on UP). Bidders must be able to prove to Piute School District's satisfaction they are indeed responsible. If we have encountered performance problems with any bidder in the past, not meeting our expectations or the specification of any bid or contract, we will not accept bids from them. Bid price must also be valid for the next-generation product if the current item listed in this bid is discontinued or the part number changes. Bidders must supply a USAC SPIN number and have a current 473 form demonstrating they are a qualified vendor. Selected Service Provider must remain current on all applicable USAC certifications throughout the life of the contract. 5. These are estimated quantities that we anticipate purchasing over the course of the contract period. Bidders must understand we may not be purchasing all of these items. 6. Bid Requirements: a) Vendor shall be a registered E-Rate Service Provider with a current form 473 Service Provider Annual Certification (SPAC) on file with USAC b) Vendor shall provide SPIN number as part of the bid. Please enter this number in the "Questions" section of the bid where prompted. c) Products must be new, not re-manufactured. Price must include freight (FOB Destination) on the ground. Any communication regarding this bid is to be conducted through the Utah Public Procurement Place (UP). Terms and Conditions are non-negotiable. Any questions regarding Terms and Conditions are to be submitted prior to bid due date. Attempts to take exception to any Terms and Conditions will render the bid nonresponsive. Any terms or conditions submitted by the bidder, with their bid, will be discarded and will not be considered Piute School District reserves the right to award the bid to the next low bid if for any reason the low bid is rejected, is disqualified or fails to perform. When submitting a bid electronically through the Utah Public Procurement Place, allow sufficient time to complete the online forms and to upload documents. The solicitation will end precisely at the closing time listed. If you are in the middle of uploading your bid at the closing time, the Utah Public Procurement Place will stop the process and your bid will not be received. A person with an outstanding tax lien in the state of Utah may not submit a bid, bid or offer. 7.
