    Description

    The City of Lake City (hereinafter, "City'') has issued this Request for Proposals (hereinafter, "RFP") with the sole purpose and intent of obtaining responses from qualified and experienced contractors to select a pre-qualified pool of contractors to provide residential and commercial redevelopment and improvement activities in support of the Community Redevelopment Agency programs. This contractor pool supports CRA grant-funded projects and does not guarantee any work assignments. Projects are subject to eligibility requirements and CRA Board approval, and no work begins until CRA approval and Notice to Proceed are issued.

    Background

    The purpose of the expanded façade grant is to enhance the appearance and encourage investment in the redevelopment area.

     

    In an effort to revitalize and further invest in the Community Redevelopment Area, the Lake City CRA has developed a comprehensive program of financial incentives to promote improvements to existing businesses for future growth and success.

     

    This grant program, for qualifying commercial and residential properties, has been designed to offer commercial and residential property owners, business owners and possible tenants the opportunity to make physical exterior improvements to vacant and/or underutilized spaces, and to encourage the adaptive reuse of and promote upgrades to existing buildings, improve appearance and environment, improve functionality, eliminate code violations, and help fill vacant and/or underutilized spaces, to foster renewed interest in opening and operating businesses within the CRA District.

    Evaluation Criteria

    • Qualifications of the Contractor - 40 Points (40 pts)

      Indicate the contractor’s experience in providing the type of services requested under this Solicitation. Licenses and any other pertinent documentation shall be submitted under this section.

      Provide at least three (3) projects similar in scope, size and complexity to the work requested by this Solicitation performed by the Respondent within the last five (5) years to include the following.

      •  Client name, address, and phone number for client’s representative
      •  Description of work & services
      •  Name team members associated with this project
      •  Year the project was completed
      •  Total cost of the project. 
    • Qualifications and Experience of the Project Manager/Project Team - 30 Points (30 pts)

      Provide a summary of the experience and qualifications of the individual(s) who will serve as project manager, as well as other team members including, but not limited to, any sub-contractors included in the Respondent’s proposal. Include copies of all licenses and certifications, if any for each of these individual(s).

    • Proposed Approach and Methodology - 10 Points (10 pts)

      Provide the following:

      a) An explanation of why the Proposer is best qualified to perform the services and demonstrate its qualifications including an item-by-item disclosure outlining how the contractor meets or exceeds the requirements of this RFP.

      b) Describe the proposers approach on how the firm intends to accomplish or achieve the scope of work under this Solicitation.

      c) Describe your firm’s understanding of the scope of work and your firm’s strengths in executing these project types.

    • References - 20 Points (20 pts)

      Respondents must provide at least three (3) successfully completed verifiable projects for which they have provided services similar to the category you applied for, within the last five (5) years. If available, such projects should be for public agencies within the State of Florida.

      I hereby certify that neither the responding entity, nor any of its wholly owned subsidiaries, majority-owned subsidiaries, parent companies, or affiliates of such entities or business associations, that exists for the purpose of making profit have been placed on the Scrutinized Companies That Boycott Israel List created pursuant to s. 215.4725 of the Florida Statutes, or are engaged in a boycott of Israel.

      In addition, if this Solicitation is for a contract for goods or services where the total contract value is one million dollars ($1,000,000) or more, I hereby certify that neither the responding entity, nor any of its wholly owned subsidiaries, majority- owned subsidiaries, parent companies, or affiliates of such entities or business associations, that exists for the purpose of making profit are on the Scrutinized Companies with Activities in Sudan List or the Scrutinized Companies with Activities in the Iran Petroleum Energy Sector List, created pursuant to s. 215.473 of the Florida Statutes, or are engaged in business operations in Cuba or Syria as defined in said statute.

      I understand and agree that the County may immediately terminate any contract resulting from this Solicitation upon written notice if the responding entity (or any of those related entities of respondent as defined above by Florida law) are found to have submitted a false certification or any of the following occur with respect to the company or a related entity: (i) it has been placed on the Scrutinized Companies that Boycott Israel List, or is engaged in a boycott of Israel, or (ii) for any contract for goods or services of one million dollars ($1,000,000) or more, it has been placed on the Scrutinized Companies with Activities in Sudan List or the Scrutinized Companies with Activities in the Iran Petroleum Energy Sector List, or it is found to have been engaged in business operations in Cuba or Syria.
