SUNY UPSTATE MEDICAL UNIVERSITY (“UPSTATE”) intends to make an award under our discretionary authority, pursuant to NYS Education Law §355(5)(a), which is up to $125,000, for a vendor to provide Primary Ambulette Transport Service for non-emergency transport of patients discharged from either Upstate University or Community Hospital to their next level of care whether it is a facility (i.e. Long-Term Care, rehabilitation, referring acute care hospital) or home environment. A sealed bid will not be held. If you are interested in providing a proposal for the above service, you must contact the appropriately designated Campus contact in this advertisement within fifteen (15) business days of this Advertised Notice. Requirements: · Must be able to do business in New York State. · The Primary Ambulette Transport Service (“Service”) must be available, five (5) days per week, Monday through Friday (including holidays), seven (7) hours per day from 10:00am to 5:00pm daily (“Designated Hours”) for a total of thirty-five (35) hours per week. · The Service must be available and stationed on site at one of the Upstate hospitals, during the Designated Hours to transport discharged Upstate patients as needed with no travel radius restriction. · Vendor’s dedicated transport unit(s) for the Service must be New York State certified ambulettes. · Vendor’s ambulette will be approved by the Department of Transportation · Vendor’s ambulette must have a cell phone or dispatch radio aboard to ensure ability to communicate with their dispatcher and Upstate at all times. · Vendor must provide two (2) qualified staff who are CPR/AED certified, one (1) to drive the Primary Ambulette Transport vehicle with appropriate NYSDMV CDL Class C licensure with a Passenger rating and one (1) appropriate patient care transport assist personnel. This procurement is subject to NYS Finance Law (“SFL”) §§139j&k, also known as the Lobbying Law, therefore, vendors are cautioned that the only permissible contact for this procurement is the individual(s) listed below. Contacting any other individuals at Upstate in connection with this advertisement may constitute a violation of SFL. Failure to comply with this requirement can range from no contract award to debarment from sales to any NYS agency. Vendors responding to this advertisement shall be required to complete and submit applicable documents related to Vendor Responsibility, NYS SFL §§139 j&k and NYS Tax Law Section 5-a.
