    The City of Clermont is seeking Statements of Qualifications from experienced architectural and engineering firms to provide professional design services for a proposed multi-level public parking garage in downtown Clermont. To address this need, the City has evaluated the development of a multi-level public parking garage and previously issued design-build solicitations for the project.         1. Provide an introduction letter outlining the respondent’s specialization, the location of the office responsible for managing the project, and a summary of the experience intended to support the respondent's qualifications to perform required professional services. The introduction letter shall be signed by an officer of the Company/Corporation submitting the proposal.
      2. License and Insurance
        1. Provide evidence that the company is registered with the state and holds an unexpired active State of Florida General Contractor’s license. Respondent shall provide proof of license indicating it is active in this section. In addition, if the respondent is a corporation, it must be properly chartered with the Florida Department of State and submit evidence of such in this section. Include a copy of the business W-9 certificate.
      3. Company Background and Length of Time in Business
        1. The respondent must have been in existence, under its current name, for at least five (5) years as an active General Contractor in good standing with a proven track record of performance and must provide evidence of such in this section. If name changes have occurred in the past Fifteen (15) years, provide these name changes in chronological order.
        2. In this section, provide a brief narrative of company history including the date the company was formed and/or incorporated, list the officers of the company submitting, and its qualifying agents.
        3. In addition, include a brief narrative of your most significant projects. Limit to no more than five (5).
        4. NOTE: The company will not be pre-qualified if the respondent has declared bankruptcy or filed for re-organization in the past ten (10) years.
      4. Staff Experience
        1. Provide the resumes of the Superintendent and Project Manager assigned to this project. Resumes must include education, years of experience, three (3) most significant projects, and any other pertinent information necessary to convey the quality of the individuals assigned to this project.
        2. The Superintendent and Project Manager must have at least five (5) years of experience operating in the same capacity (position) and working on projects of similar scope and size. The Superintendent and Project Manager must be an employee of the respondent for the last two (2) years.
        3. If available, provide letters of recommendation from architects and/or owners that identify these individuals as being instrumental in the success of the project they were on.
      5. Project Experience
        1. Submit descriptions and final project photos of at least five (5) completed projects of similar type, size, and complexity. Experience should include at least one (1) project that was an Administration Building and one (1) project that included a Vehicle Maintenance Building completed by the respondent. Include project details such as project name, project location, size in square feet, features similar to the proposed project, the project owner, owner contact person, his/her phone number and email address, the project cost (including original contract value and final contract value), and any other significant features associated with the work.
        2. Projects listed in this section should have been completed by the respondent's company within the past ten (10) years.
      6. Subcontractors and Owner References
        1. The General Contractor must provide three (3) subcontractors and three (3) owner references who can be contacted to validate the General Contractor’s experience and past performance. Include the contact name, the project name worked on with the sub or owner, and their phone number (ensure that the number is current and the person you are using as a reference is still working for the entity on the reference).
        2. Include three (3) letters of recommendation from Architects and/or Owners (other than those identified above) from projects that the respondent has completed within the past five (5) years.
        3. Provide a list of the major subcontractors you contract with on a regular basis and would most likely solicit bids from on this project. Understanding that this is a hard-bid project, submitting the list will not be considered a commitment from the respondent to contract with any of the subs listed.
      7. Bonding Information
        1. Submit evidence (in the form of a notarized letter from the surety) indicating the firm’s ability to provide 100% payment and performance bonds for this project by including a statement of available and aggregate bonding capacity from a surety company authorized to practice in the State of Florida by the Department of Insurance. The surety shall have an A.M. Best rating of B+ or better with a Financial Size Category (FSC) of VI or better. The certificate shall provide A.M.'s Best number for each surety.
