    Description

    1. The City is accepting Statements of Qualifications (SOQ/proposal) from Contractors related to the repair and rehabilitation of single-family housing units within the Pueblo city limits. This initiative is funded through a number of sources, but primarily through an allocation of Community Development Block Grant (CDBG) and HOME Investment Partnership Funds provided by the U.S. Department of Housing and Urban Development (HUD), and State of Colorado Proposition 123 funds. Jobs will typically fall under the requirements of the City of Pueblo's Minor Home Repair Program of $ 10,000, though can be as much as $ 25,000.
    2. Contractors must identify which trade(s) they are applying for as outlined in Section 2.5 of the PROPOSAL FORMAT AND REQUIRED RESPONSES. The City will maintain a separate list of qualified Contractors in each trade for a minimum period of two (2) years. Contractors determined to be qualified will be invited to bid on future projects as they become available.
    3. Note: This Request for Qualification is not intended for environmental consulting or abatement firms. While limited abatement of environmental hazards may be required as part of the certain propjets, this RFQ is specifically seeking Contractors to perform rehabilitation work.

      By confirming below, the Proposer, having carefully read and considered the Request for Qualifications (RFQ) for the above referenced project, does hereby offer to provide such goods and services on behalf of the City in the manner described and subject to the terms and conditions set forth in the attached RFQ. All goods and services shall be provided at the rates set forth in submitted proposal or as negotiated by all involved parties.

      Proposer acknowledges that the company is qualified to provide these types of goods and services. At any time during the selection and award process, the City may request information substantiating the indicated requirements. Failure to provide this information may result in a proposal being declared non-responsive. Proposer acknowledges and accepts that all components of and responses to this RFQ will be included and become a part of the final agreement by reference.

      By confirming below, the Proposer further states that this Proposal is made in good faith and that the prices offered were independently developed and are not founded on, or in consequence of, any collusion, agreement or understanding between themselves or any other interested party.

    • Waiver and Release (required)

      The City may conduct such investigations as the City considers necessary to assist in the evaluation of any proposal to establish the responsibility, qualifications, and financial ability of any potential Proposer to perform the services specified under this RFP within the prescribed time. By submitting a proposal, the Proposer authorizes the City to obtain information concerning the Proposer’s performance on other projects it has completed during the prior ten (10) years, including those identified in the submission and those not so identified, of which the City may become aware. By submitting its proposal, the Proposer and subconsultants waive and release all claims against owners, architects, and engineers, and their agents and representatives, relating to or arising from the furnishing of such information to the City concerning the Proposer’s performance on prior projects. In order to effectuate the intent of this clause, each Proposer may be required by the City to execute information release authorization forms, which specifically release all information providers from all claims that arise from or relate to the information provided.

      • Has not been convicted under local, state, or federal antitrust statutes arising out of the submission of bids or proposals.
      • Has not failed to perform or unsatisfactorily performed in accordance with the terms of one or more contracts with public or private entities within the previous five years.
      • Is not currently under suspension or debarment by the City or any other governmental entity based upon a settlement agreement or a final administrative or judicial determination issued by a federal, state, or local governmental entity.
