    Description

    The Hawai'i County Office of Aging (HCOA) is planning to procure Case Management Services for older individuls (60 years of age and older) who reside in Hawai'i County. Contracts are planned to begin on or after July 1, 2026. Seeking applicants to provide Case Managment Services for Elder individuals on Hawaii Island. Case management services would provide assistance in the form of access to services and case coordination for qualified individuals experiencing diminished functioning capacities, personal conditions, or other characteristics which require the provision of services by compensated service providers. The purpose of this Request for Information (RFI) is to assist HCOA in obtaining community input for the development of a Request for Proposals (RFP) for person-Centered Case Management for our elder population and Person-Centered Case Management for Family Caregivers. HCOA is specifically interested in feedback and comments regarding the items listed below.However, we welcome additional comments and suggestions. 1.. Support Plans & Expectations 2.
