    Description

    The purpose of this Colorado Department of Public Health and Environment (CDPHE) Request for Proposals (RFP) is to solicit responses from a qualified Offeror to provide the statewide implementation and evaluation of an innovative, incentive-based tobacco/nicotine and/or cannabis cessation program specifically created for pregnant and postpartum Coloradans and their partners. The program services will be available statewide via two options: local in-person visits or a Colorado-based telehealth model. All facilitators of the program will be supported in obtaining the requirements for the Community Health Worker (CHW) certification, as well as a trauma-informed curriculum. Financial incentives for program participation will be provided. Evaluation of the program will be conducted, and results will be published.
