Providing access to home health services allows University Health to continue working on throughput with decreasing length of stay and decompressing the emergency department because we can safely discharge patients to their homes with follow up care provided by skilled staff.University Health will award a three (3) year contract with two (2) one (1) year renewal options to a prime contractor who will be responsible for the complete oversight and management of all the components of Home Health Care Services for designated University Health patients within Bexar County and surrounding counties. Services required are outlined in this Request for Proposals. Prime contractors may engage in and manage subcontracts to ensure that the objectives of the contract are met. Prime contractors shall be solely responsible for the management and oversight of their subcontractors and ensuring Contract compliance.All awardees will be invited to participate in the University Health High Value Post‑Acute Network (HVPAN) and will be required to sign the corresponding agreement.All awardees must also be willing to contract with CareLink should they be requested to do so. Note, CareLink contract to be negotiated separately.
